Goal: The goal of Sojourner Health Clinic is to pick up where the current medical system leaves off by providing free acute and ongoing healthcare to patients who do not have access--or are reluctant to access—traditional routes to a doctor or medicine.

Impact: During 2012, the Sojourner Health Clinic managed to increase the number of diabetic patients with A1C levels below 9 percent from 15% to 83%, reducing the average blood glucose level of patients and giving them a better sense of control over their diabetes.

Goal: The Early Childhood Cavities Prevention Program (ECCPP) is a community-based intervention program to promote preventive oral care for both mothers and their infants in Klamath County, Oregon.

Impact: The Early Childhood Cavities Prevention Program (ECCPP) educates pregnant women about dental hygiene to prevent dental infection in their children. Ninety-three percent of all participating infants were 100% cavity-free on their second birthday.

Goal: The main purpose of the Eat Well! El Paso program is to improve the local food environment by expanding healthy food options for children and their families, as well as to increase awareness and knowledge of the positive effects that healthy food choices can have on our health and our community.

Impact: Many locally-owned restaurants in El Paso have worked with the Eat Well! El Paso program to improve the healthy meal choices on their menus, helping to create a healthier food environment for the community.
