The overall goal of Healthy Futures is to improve community health by ensuring access to health care and community resources for pregnant women and young children in the region.
In 2012, over 3,500 contacts, 970 home visits, and 30,000 educational mailings were delivered. Of those who initiated breastfeeding at birth, 67% of enrolled infants are still breastfeeding at eight weeks post-partum. Eighty-three percent of enrolled two-year-old children are fully immunized.

The program aims to reduce childhood obesity by using a three-stage approach to identifying root causes of the issue, developing and implementing behavioral strategies for weight management, and involving both the children and their families in adopting a healthy lifestyle.

Healthy Howard opens doors for individuals, groups and communities to improve their health and wellness.
Healthy Howard provides resources to schools, restaurants, workplaces, and childcare centers to help them improve the health and choices of their students, employees, or clients.

The mission of the Healthy Kids Express Asthma Program is to provide quality asthma care and education to children with asthma and their caretakers in a school environment to work towards goals set by Healthy People 2020 and the National Asthma Education and Prevention Program (NAEPP).
The Healthy Kids Express Asthma Program serves approximately 600 children a year, improving inhaler technique and asthma knowledge among participants. Children enrolled in the program for two consecutive years have lower school absentee rates and hospitalizations due to asthma.

The goal of Healthy U is to work with several community groups to provide healthy snacks as well as education on nutrition and fitness to students and their families at University Academy.
The percentage of screened students who were overweight fell from 45.9% in the 2011-2012 school year to 36.9% in the 2011-2012 school year. Over 75% of participants in both Teen Eats and Family Dinners demonstrated a positive behavior change.

HeartStrings mission statement is as follows:

familiar setting.
3) To provide a valuable resource for facilities that serve aging populations, adults with dementia, and individuals with disabilities, or long-term illnesses.
HeartStrings has reached over 3,200 individuals with disabilities, long-term illness, and assisted-living needs, as well as aging adults with dementia since its start in 2006.

The goal of Hep Free Hawai'i is to offer hepatitis vaccinations, encourage Hawaiians to learn of their hepatitis status through testing, and connect individuals with hepatitis or liver disease to medical resources and support groups.

To educate the Chinese Canadian immigrant community on Hepatitis B knowledge and to promote Hepatitis B testing through ESL curriculum.
The Chinese immigrant population in North America has a rate of Hepatitis B infection that is 10 times higher than the general population's rate of approximately 0.5 percent. The Hepatitis B ESL curriculum is effective in increasing knowledge about Hepatitis B among Chinese immigrants in Canada.
